Managing chronic conditions like diabetes, hypertension and high cholesterol often times requires medications along side the right nutrition, as provided in our MNT programmes. So to ensure you are getting the very best out of these medications and minimizing side effects, we offer Medication Therapy Management Services (MTMS).

MTMS is a group of personalized services provided by pharmacists, that help you get the best out of your medications, ensuring they are effective, safe and saving you unnecessary healthcare costs. This means we are able to ensure your blood pressure, blood sugar and blood cholesterol are well controlled while keeping you safe. We provide the following services under our MTM programme:

Medication Therapy Review (MTR)

This service involves our pharmacists going through all your medications, either prescribed, over-the counter, supplements or herbals, to detect drug therapy problems, prevent potential ones and resolve them.

Drug therapy problems can include:

  • Over and under-dosage
  • Drug interactions
  • Wrong drug use
  • Need for additional drug therapy
  • Unnecessary drug therapy
  • Adverse drug reaction
  • Inappropriate compliance

Drug therapy problems that are detected are resolved with your collaboration and that of your physician if the case arises.  The problem(s) would be explained to you, so you understand the changes that might ensue. If your physician is required to make the changes, we relay the problems to him/her, explaining our findings and our suggestions towards resolving them.

The benefits of this service include the following:

  • better managed health conditions, which reduces the risk of developing complications.
  • safer use of medications.
  • saves cost both short and long term. Short term saving can be through discontinuation of unnecessary medications or switch to a more cost-effective brand of a medication. The long term savings are through the prevention of complications and their associated costs, including the physical and psychological burden.

To help you use your medications properly, and to help your doctor prevent some of the drug therapy problems mentioned above, such as duplicating medications and drug interactions, we provide you with a Personal Medication List (PML)

The PML is a simple document that lists all of the medications you are currently taking, including the prescribed, over-the-counter and supplements. It contains information about,

  • How to take and when to take them
  • Why you are taking them 
  • Any special notes concerning their use
  • Common side effects you may experience and 
  • More serious side effects that would require you visit the hospital or call your doctor.

Disease State Education

Managing any health condition requires that you have adequate knowledge about it, to guide you in making decisions that affects it. We have noticed a great lack of knowledge or misconceptions individuals have about the condition they are managing, and this has sometimes affected their management.

We help you understand your conditions and identify the roles you have to play in managing it. We do this through counselling and providing you with very easy to understand and illustrative materials. You are a part of your healthcare team, and with proper education we help you assume your role.

Health and Wellness Screening

The outcome of any condition is highly based on how early it is detected, and subsequently managed.


We offer regular body mass index (BMI), waist circumference, blood pressure and glucose checks; and when the need arises we would ensure you get other necessary tests  done as at when due.

Drug Information


To ensure that your treatment regimen remains safe and effective, your personal pharmacist would be available to you to answer questions you might have concerning any medications you are taking or plan to take, to avoid any of the drug therapy problems mentioned earlier.
